Can I get a replacement ID or Driver’s License in your office?
What documents do I need to obtain an ID For the first time?
- Birth Certificate or Current Passport
- SSN Card
For a Travel ID, you need:
- Birth Certificate or Current Passport
- SSN Card
- Two forms of proof of address. These will need to show your name and current address
What are the application requirements for Driver's Licenses and IDs?
- You must be 16 years or older.
- You must complete a Driver License/Identification Card application (AZdot.Gov).
- You must provide proof of identification.
- You must pass the vision, and/or written and driving tests. Practice tests are provided below.
- You must provide proof of residency and presence.

What if I have more questions?
Teenage driver looking to take the written driver test to get an Instruction Permit can now take that test online with appropriate supervision.
Permit Test @ Home is available for parents or legal guardians to administer after setting up their free, personalized account on MVS’s new portal, AZ MVS Now (azmvdnow.az.gov).
Note: Teenagers’ accounts cannot be used for taking the test. The parent or legal guardian must have an account.
Steps for the parent or legal guardian:
- Login or create your AZ MVS Now account
- Verify your identity
- Complete your teen’s information
- Pay the $7 application fee
- Supervise the test
Participating Arizona Professional Driving School trainers can also administer the test on behalf of the parent or legal guardian.
It is recommended that teens study the Arizona Driver Manual(link is external) and take the online practice tests before attempting the Permit Test @ Home.
Note: At this time, Permit Test @ Home is offered only to teenagers under 18.
